FORMS OF PROOF REQUIRED FOR MOTOR VEHICLE CREDIT OR REMOVAL

Note: All forms of proof must specifically identify the vehicle in question by make, year and vehicle identification.
VEHICLE SOLD – NOT REPLACED (2 Forms of Proof)
REQUIRED - Receipt from the Motor Vehicle Department for returned marker plates.
Additional forms of proof:
1.Letter from the insurance company indicating the date that the vehicle was sold. This letter must include make, model, and vehicle identification number.
2.A copy of the bill of sale. If sold privately and not to a dealership, the bill of sale must be notarized.
3. Copy of original registration of the person to whom you sold the vehicle.
4.Copy of the vehicle’s transfer of title.
5. Receipt or letter from the salvage yard and/or dealer showing the date of disposal.
Note: Two forms of proof may substitute for a lost plate receipt.
VEHICLE TOTALLED (2 Forms of Proof)
REQUIRED: Receipt from the Motor Vehicle Department for returned marker plates or plate cancellation.
1.Letter from the insurance company stating that the vehicle was totaled, and the date when this occurred.
2.Receipt or letter from the salvage yard and/or dealer showing the date of disposal.
VEHICLE REGISTERED OUT OF STATE
1.Copy of the motor vehicle registration from the state where it is now registered, and a copy of his/her out of state driver’s licenses.
VEHICLE STOLEN
1.A statement from your insurance agent stating the date of the theft, year, make, model, vehicle identification number, and that the vehicle was not recovered.
VEHICLE REPOSSESSED
1. A copy of Connecticut Department of Motor Vehicles cancellation of plate receipt.
2. Letter from the finance company stating the date that the vehicle was taken and that it was not redeemed by you. This letter must include make, model, and vehicle identification number.
